Job Description
Sheffield City Council is recruiting four full-time Provider Support Assessors to join the Home First Community service (recently rebranded from the Short Term Intervention Team), based at Manor Centre, Sheffield. This is a citywide, field-based role focused on hospital discharge support and short-term community care for predominantly older adults aged 65 and over.
Working Monday to Friday between 08:00 and 20:00 (8.5-hour shifts with a paid break), you will manage daily referrals via the LASS (Liquidlogic) system, conduct initial assessments, review visits, and mobility and environmental risk assessments, develop support plans, undertake medication checks, and maintain accurate records. The team of 16 Assessors works collaboratively but each holds individual caseload responsibility.
The service welcomes candidates from a range of practical care backgrounds including social care, community nursing, domiciliary care coordination, or experienced support workers ready to step up. Experience is prioritised over formal qualifications — Level 2 or 3 qualifications are not essential. A structured induction includes shadowing, moving and handling, and medication training.
A full UK driving licence and access to your own vehicle are essential, as is a willingness to travel citywide. You must be resilient, confident, highly organised, and comfortable working autonomously in a fast-paced environment.
Rate: £17.18 per hour. Contract: 29 June 2026 to 28 December 2026. Enhanced DBS required.
