Job Description
Flintshire County Council is seeking a qualified Occupational Therapist to join their Social Services directorate, based at Tŷ Draig (formerly Ty Dewi Sant), St David’s Park, Deeside, with work across the county. This is a community-facing role carrying a caseload of complex cases across people’s own homes, sheltered accommodation, residential settings, and schools.
You will complete outcome-focused OT assessments — including manual handling, seating, and environmental assessments — and formulate person-centred action plans that build on individual strengths. You will assess and recommend disability equipment and minor or major home adaptations in collaboration with housing partners, enabling clients and carers to maintain independence and safety in daily living. Where adaptations are not appropriate, you will support re-housing assessments.
Close partnership working is essential: you will liaise regularly with colleagues across health, social care, housing, and the third sector, and produce timely, accurate case records in line with professional governance and data protection requirements.
Essential requirements: a degree in Occupational Therapy and current HCPC registration. Knowledge of the Social Services and Well-Being (Wales) Act is expected. Experience of providing services in people’s own homes using a strengths-based, enabling approach is required.
