Job Description

Sefton MBC is seeking a qualified Social Worker to join a Young People’s Social Work Team in Bootle, Merseyside, supporting children in care aged 16–25 through transition into adulthood. This is a statutory leaving care role covering pathway planning, risk assessment, and intervention for eligible, relevant, and former relevant young people under leaving care legislation.

You will hold a caseload of young people with complex needs — including those affected by exploitation, mental health difficulties, offending, homelessness, substance misuse, and family breakdown — ensuring their Pathway Plans are analytical, outcome-focused, and genuinely reflective of their wishes and feelings. A key part of the role is coordinating transition planning with Adult Social Care, housing, health, and education partners, and supporting young people to develop the practical independent living skills they need for adulthood.

Candidates must be Social Work England registered and have demonstrable experience in children in care or leaving care services. Experience of working with vulnerable young people aged 16–25 and knowledge of statutory responsibilities towards looked-after and care-experienced young people is essential.
