Job Description

A qualified Social Worker is required by the Royal Borough of Kingston to join their Mental Health, Learning Disabilities & Transitions team, based at Guildhall 2, Kingston upon Thames (KT1 1EU). This is a temporary role paying £22.21–£29.86 per hour via Prism, starting 31 August 2026.

The role centres on reviews and reassessments, including progression through the Continuing Healthcare (CHC) Pathway. You will hold a caseload of adults with complex needs, conducting Care Act assessments, developing outcome-focused support plans, and undertaking financial assessments. Work involves direct engagement with individuals, families, carers, providers, and brokerage to agree changes to care and support arrangements.

Key responsibilities include accepting and screening referrals, applying Care Act eligibility criteria, carrying out carer assessments, presenting cases at the Quality and Risk Forum, and completing Resource Allocation System assessments. You will raise and progress safeguarding concerns in line with Making Safeguarding Personal principles, and provide supervision and coaching to non-qualified staff. The role reports to a Team Leader and requires close working with health partners and external providers.

Applicants must hold a recognised social work qualification, be registered with Social Work England, and have demonstrable experience in adults’ services — particularly mental health, learning disabilities, or transitions work. CHC pathway experience is advantageous.
