Rochdale Adult Care (CQC rated Good) is recruiting a qualified, enthusiastic Hospital Discharge Social Worker to join the Home First Hospital Discharge Hub within the Integrated Middleton Locality. This is a fast-paced, dynamic role at the heart of adult care transformation, working alongside district nurse and health colleagues across hospital sites to support residents on discharge and promote independent living.
You will conduct Mental Capacity Assessments and Best Interest processes, apply strength-based approaches to discharge planning under the Care Act 2014, and work within tight timescales to achieve positive outcomes for adults and their carers. The role requires visiting hospital sites local to Rochdale and occasionally further afield — access to a car is essential. The post is primarily office-based at Brook House, Middleton, with some opportunity for home working, and includes duty responsibilities.
Requirements: qualified Social Worker registered with Social Work England; current hospital social work experience; strong knowledge of the Mental Capacity Act; excellent communication, organisational, and ICT skills; 2 years of checkable references with gaps in employment history verified.
Rate: £30 per hour. Fixed-term: 27 July – 27 November 2026. Based at Brook House, Oldham Road, Middleton, M24 1AY.
