Job Description
An exciting opportunity has arisen for an experienced Social Worker to join a busy Hospital Social Work Team based at Manchester Royal Infirmary, one of the UK’s leading NHS acute trusts in central Manchester.
Working within Adults Services, you will sit alongside ward staff, therapists and discharge coordinators to support safe and timely discharges for adults and older people with complex needs. This is a fast-paced, high-impact role where your assessments and decisions directly enable people to leave hospital with the right care in place.
Key responsibilities include carrying out Care Act assessments and reviews on the wards, coordinating hospital discharges under Discharge to Assess (D2A) pathways, and liaising with community teams, reablement services and brokerage. You will undertake safeguarding enquiries under Section 42 of the Care Act, complete Mental Capacity Act assessments and Best Interest decisions, and refer for DoLS where appropriate. You will manage a varied caseload with a strong focus on throughput and quality, working closely with NHS colleagues, families and care providers to plan residential, nursing and domiciliary care arrangements.
To be considered, you must hold a recognised social work qualification (DipSW, BA or MA Social Work) and be registered with Social Work England. Post-qualified experience within an adults hospital or discharge team is essential, or demonstrable hospital-facing work within a community adults setting. You must be confident applying the Care Act 2014 and MCA 2005 in an acute environment and hold an enhanced DBS.
