An exciting opportunity has arisen for an experienced Social Worker to join the Hospital Social Work Team based at Manchester Royal Infirmary, one of the UK’s leading NHS acute trusts in central Manchester.
Working embedded within the hospital, you will play a pivotal role in facilitating safe, timely discharges for adults and older people with complex needs. You will collaborate daily with ward staff, therapists, discharge coordinators, community teams, reablement services, and brokerage to ensure seamless transitions from acute care to appropriate community or residential settings.
Key responsibilities include carrying out Care Act assessments and reviews on the wards, coordinating discharges under Discharge to Assess (D2A) pathways, undertaking safeguarding enquiries under Section 42 of the Care Act, completing Mental Capacity Act assessments and Best Interest decisions, and referring to DoLS where appropriate. You will manage a varied caseload with a strong focus on throughput and quality, working alongside NHS colleagues, families, and care providers to plan residential, nursing, and domiciliary care arrangements.
To be considered, you must hold a recognised social work qualification (DipSW, BA or MA Social Work) and be registered with Social Work England. Post-qualified experience within an adults hospital or discharge team is essential, or strong community adults experience with demonstrable hospital-facing work. You must be confident applying the Care Act 2014 and MCA 2005 in a fast-paced acute environment and be eligible to work in the UK with an enhanced DBS.
