Job Description

An experienced Senior Practitioner is sought to join Herefordshire’s CAAsT Locality Team in Adult Social Care, based at Plough Lane, Hereford. This is a locum opportunity offering up to £32.67 per hour (umbrella) on an initial 3-month contract, working 37 hours per week, with a start date of 7th May 2026.

In this role you will hold a small caseload across a broad spectrum of adult service users, including those with mental health needs, learning disabilities, physical disabilities, frailty, and safeguarding concerns. You will work within the locality management team to support performance and lead practice, making evidence-based, defensible decisions in a supportive environment that champions strengths-based and asset-based approaches under the Care Act 2014.

Key responsibilities include undertaking person-centred, outcome-focused assessments and care management reviews, providing supervision and line management support, and contributing to the professional development of the wider team. Herefordshire is a rural county, so the ability to travel across the area is essential.

To be considered, you must be registered with Social Work England, hold a recognised Social Work qualification, and have a minimum of three years’ post-qualifying experience in adult social care. Supervisory or line management experience is expected, along with a sound knowledge of adult social care legislation and community asset-based practice.
