Job Purpose:

Promote independence and reduce risks for Adults with disabilities living in Doncaster. Work with the social model of disability and use evidence based practice to carry out person centre assessments to identify need and take appropriate action. Use person centred methods that respect individual values, cultures and diversity to identify needs and devise individual interventions to meet need. Where statutory provision is not appropriate offer information and advice around self-directed support.

There is a requirement to provide a high quality, evidence-based, Integrated Community Equipment and Adaptations Service to people in their own homes which will be achieved by carrying a complex caseload and using a robust performance management framework to ensure effective service delivery by all officers working in the service.

This service is target driven with a clear focus on activity and performance. The post holder is required to meet performance targets, actively participate in the data capture process and report to the Deputy Principal OT / Principal OT when targets are not met.

To provide expert clinical knowledge on manual handling issues and support other colleagues with the most complex manual handling cases.

Main Duties and Responsibilities

Carry out needs led assessment of occupational performance with Clients who have a wide range of medical conditions and physical disabilities. Acknowledge and analyse physiological, psychological and environmental factors to develop individual goal orientated action plans.

Take into account the views of the person and their carer(s) to agree appropriate provision of equipment, minor and major adaptations or re-housing that is necessary to meet current and long-term needs whilst considering the legal framework, feasibility, practicality and resource implications of the proposed plan. Have particular responsibility for people with the most complex needs.

This post will have particular clinical responsible as locality lead to work across organisational boundaries to deliver integrated housing solutions to meet the long term needs of the most complex Adults. This will involve innovative solutions outside of traditional solutions to ensure best value in the longer term.

Carry out holistic assessments and take action to reduce and manage risk to people in their homes advising them and their carer(s) on various aspects of safe independence or care. Work in collaboration with assessment and care management to ensure care provision is appropriate post intervention i.e. care package is reduced if independence is increased.

Work autonomously to manage own complex clinical caseload. Provide clinical advice to cases managed by other Occupational Therapists and Assessment Officers. Recognise specific areas of concern and the need to re-prioritise cases according to circumstances or to refer people to other services and agencies.

Provide expert advice and intervention regarding complex manual handling situations. This may involve joint working with colleagues and providing advice/ training to care agency staff. There is a requirement to ensure appropriate capacity is in place for manual handling training.

Participate in triaging referrals to identify eligible need and priority. There is a requirement to support the role of the Adult Contact Team/ Single point of access with regards self-assessment and direct provision of services. Signposting and advice to clients who do not meet the criteria of council service provision or do not wish to have a service provided but wish to provide this themselves.

Promote the use of assisted technology, including Telecare to encourage independence

Undertake assessments and documentation associated with Blue Badge applications

The OTs have particular responsibility to work in partnership with St. Leger Homes Doncaster around the management of the accessible housing register. This involves not only matching the needs of the person to the vacant adapted property, but also ensuring that the property is offered to the person with the highest level of need. This may also include the need for a vacant property check to ensure the property is suitable.

Have and maintain a current working knowledge of the safe use of a wide range of Community Equipment. Act as an Equipment Authoriser and be responsible for providing appropriate clinical challenge for prescribers across health and social care.

Keep clear, concise, accurate records according to professional standards and local policy and ensure compliance by other members of the team. Regularly record statistical information to provide information needed for Performance Indicators.

Responsible for reviewing and reflecting on own practice and performance and participating in the Council’s appraisal process.

Provide occupational therapy practice education and be responsible for the induction and training of students on fieldwork practice placements.

To deputise for the Deputy Principal OT when required.

Undertake other duties as appropriate to the post and grade as required by the Teams Management.
