*Adults Social Worker – Prevention Service – #5302705

Job Description

Lancashire County Council is seeking a qualified Adults Social Worker to join the Prevention Service, based at Thornton Library in Fylde and Wyre (FY5 3SZ). This is a permanent, full-time post of 37 hours per week, Monday to Friday, paying £33.00 per hour, with interviews being arranged as soon as possible.

In this role you will complete Care Act assessments and participate in Multi-Disciplinary meetings to consider eligibility for Continuing Health Care for individuals discharged from hospital with Discharge Support funding. Service users will typically be in a bed-based setting, and you will work to defined timescales, promoting strength-based, holistic interventions that focus on what matters most to each individual.

You will engage with service users, their carers, and system partners to undertake assessments virtually and to commission appropriate care that promotes independence and meets identified needs, supporting the Discharge to Assess model throughout.

Applicants must hold a recognised social work qualification and be registered with Social Work England (or have applied to register). You will bring enthusiasm, flexibility, and a commitment to evidence-based, creative practice in a dynamic environment.
