Job Description

Caerphilly County Borough Council is seeking a qualified Social Worker to join the Adult Services – Older People section, based at The Rhymney Integrated Health and Social Care Centre, Rhymney, Caerphilly (NP22 5PW). The role is offered via Prism at £30.00–£39.58 per hour, starting 6 July 2026.

Working within the Assessment and Care Management Team, you will deliver person-centred assessment and care management services to older adults, maximising independence and dignity. Key responsibilities include completing needs and risk assessments, developing and reviewing care and support plans, responding to crisis situations, and contributing to safeguarding and multi-disciplinary working alongside health colleagues, community nurses, and commissioning partners.

The post requires registration as a Social Worker with Social Care Wales (or an equivalent regulator in England, Scotland, or Northern Ireland). A Diploma or Degree in Social Work is essential, along with knowledge of the Social Services and Well-being Act (Wales), Mental Capacity and Deprivation of Liberty legislation, and assessment processes for adults and carers. Experience of assessment, care planning, and community-based practice is required. A full UK Category B driving licence and access to a vehicle insured for business use are essential. Welsh language skills are desirable.
