Job Description
Newport City Council is seeking an experienced Adults Social Worker to join one of four NCN Managed Care Teams, based at Newport’s Civic Centre (Godfrey Road, NP20) with regular community-based working across Newport.
Working within a strengths-based framework aligned to the Social Services and Well-being (Wales) Act, you will undertake assessments, reviews, and short-term pieces of work to maximise independence and well-being. Caseloads span adults of all ages in their own homes, residential and nursing placements, and supported living settings. You will collaborate closely with older adult mental health teams, learning disability services, and physical disability services.
Key responsibilities include managing a caseload, completing strengths-based assessments and risk assessments, maintaining accurate documentation and electronic records, and working in partnership with families, carers, and external organisations.
The successful candidate will have statutory managed care experience, strong written skills, and the ability to manage time effectively across a varied caseload. A driving licence is required, as the role involves community visits. The post is based on-site in Newport up to five days per week depending on service need.
