Rochdale Adult Care are recruiting for an experienced social worker to join the Middleton Integrated Neighbourhood Team to cover maternity leave of the current post holders.

We work across friendly, dynamic teams alongside our district nurse colleagues with a focus on partnership working to achieve best practice and desired outcomes for our residents. We take pride in everything we do to ensure our services continue to be of an excellent and high quality standard.

 

What are we looking for?

We are looking to appoint 1 full time social worker Monday – Friday 8.30 am – 4.45 pm and 1 social worker Tuesday – Friday 8.30 am – 4.45 pm

You will be an experienced positive and enthusiatic social worker who promotes a strength based approach to assessments, support planning and offering advice and guidance to colleagues, service users and carers, as within the objectives of the Care Act 2014. Additionally, social workers are expected to promote the independence and well-being of service users and carers, utilising natural support and the voluntary sector, ensuring creativity with solutions to support planning

You will be experienced in compleing Mental Capacity Assessments & Section 42 Enquiries

You will be expected to support the Home First Hospital Discharge Hub when there is social work pressures for discharge.

You will be expected to be part of the Duty Rota

You will be an effective multi-tasker with excellent organisational skills and the ability to work flexibly to meet the needs of the service.

You will be motivated and forward thinking, who can demonstrate professionalism and enthusiasm

You will be a team player

You will have excellent interpersonal and communication skills

You will have excellent ICT skills and be able to use a range of packages including Word, Excel and Outlook.

You will have a real attention for detail and be able to maintain accurate and detailed records using both manual and electronic systems. Experience in the use of liquid logic is beneficial

There is an opportunity to work from home but this is as the needs of the service allows.
