Job Description

Experienced social worker required for a Hospital Discharge Team role based at Northlands Community Health Centre, Sheffield, supporting adult inpatients with a mental health background. Core duties include undertaking social care assessments and delivering timely care packages for hospital inpatients prior to discharge, mental capacity assessments and best interest decisions, safeguarding as required, and working within Section 117 aftercare requirements including attending S117 meetings, ward rounds and professional meetings. The role includes participation in a duty rota. A car driver is preferred, along with previous knowledge of LAS (Liquid Logic).
