Job Description

Join a fast-paced multidisciplinary team within Manchester’s HDTA Hospital Service, supporting citizens discharged from Central Manchester hospitals into bedded care or back into the community. You will complete comprehensive strengths-based assessments, undertake Mental Capacity Act Assessments, coordinate Best Interest Decisions, and lead on Safeguarding Investigations, while participating in the team duty rota. Working closely with health colleagues, you will manage a busy caseload requiring strong critical thinking, risk management and communication skills. The team offers regular formal and informal supervision within a supportive multidisciplinary environment. Candidates must be a qualified, experienced Social Worker registered with Social Work England, confident working at pace and collaborating with health and social care professionals.
