Job Description

Bolton Council (GM Collaboration) is seeking a Start Well Locality Practitioner based at Great Lever Best Start Family Hub, Leonard Street, Bolton, BL3 3AP. This is a temporary role running from 8 June to 28 August 2026, paying £16.08–£17.85 per hour PAYE (£20.39–£22.63 umbrella).

Working within the Start Well service, you will support parents and carers with a focus on improving and influencing the home learning environment before gaps in child development or emerging needs escalate. Key duties include completing home visits for children aged 18 months, carrying out WellComm assessments, discussing and completing the ASQ-SE with families, and providing tailored support and advice. Revisits are arranged based on identified needs, and accurate recording of visit outcomes is required both internally and on the NHS Foundation Trust’s Electronic Patient Record.
