Halton Borough Council is seeking a committed and compassionate Social Worker to join our Intermediate & Frailty Team. This is an exciting opportunity to support some of our most vulnerable residents, helping them maintain independence, improve well-being, and safely manage complex health and social care needs.

About the Role

As a Social Worker within the Intermediate & Frailty Team, you will:

Work closely with older adults and individuals with frailty and complex needs.

Carry out strengths-based assessments and develop effective care and support plans.

Collaborate with multi-disciplinary partners including health professionals, hospital teams, community services, and voluntary organisations.

Support safe hospital discharges and prevent avoidable admissions.

Undertake Mental Capacity Act assessments and safeguarding enquiries where required.

Promote independence, resilience, and high-quality outcomes for adults and their families.
Please be aware this is a Long Term Temporary post to start ASAP.

You must have:

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
