Job Description
An exciting opportunity has arisen for an experienced Social Worker to join the Acute Adults team based at Wyre CAPSS in the Fylde and Wyre area. This is a meaningful role at £35.00 per hour, available from 5th May 2026.
You will provide a professional statutory social work service within an acute setting, working to deliver strengths-based, outcome-focused support that promotes independence and enables recovery. A key element of the role involves working collaboratively with health colleagues to ensure safe and timely hospital discharges, matching individuals with the most appropriate services and settings to support their choice and autonomy.
You will manage complex cases, liaising with a wide range of professionals and agencies to address multifaceted needs and achieve the best possible outcomes for individuals and their carers.
To be considered, candidates must hold a recognised Social Work qualification, be registered with Social Work England, and have a minimum of three years’ experience in a permanent post within a local authority. Candidates must not have held a permanent or fixed-term position with a local authority within the last 12 weeks.
