Job Description

A unique opportunity has arisen to join a small team who are committed and passionate about improving the lives of children/young people with disabilities, complex health needs or chronic illness.
Working in the community you will be responsible for assessing the needs of children/young people and recommending appropriate interventions, including specialist equipment and major adaptations and demonstrating to families and carers alternative ways of carrying out daily living tasks, to promote and maintain children/young people’s independence.

You will need to have a good working knowledge of relevant legislation, sound assessment and problem solving ability, with good computer skills.

You will work independently, be a good team player and have proven organisational skills.

Experience of working with children and assessing for equipment and major adaptations is essential

This is a full time role- 36 hours per week.
