Job Description
Herefordshire Council is seeking a qualified Adults Social Worker to join the Young Adults (Transitions) Team in Hereford, supporting young people aged 18–25 as they transition from children’s into adult social care.
Working in line with the Care Act 2014 and Mental Capacity Act, you will complete strengths-based assessments, identify eligible needs, and develop person-centred care and support plans that promote wellbeing and independence. The role encompasses Section 42 safeguarding enquiries, Continuing Healthcare (CHC) assessments, Mental Capacity Act assessments and best-interest decision-making, and contributing to court work as required. You will work closely with young adults, their families, and multi-agency partners to ensure safe and positive transitions into adult services.
This is a locum post based at Plough Lane, Hereford (HR4 0LE), paying £23.33–£31.97 per hour via Prism. Available from 3 August 2026.
