Job Description

Somerset Council is recruiting a qualified Adults Social Worker to join the Transition to Adult Social Care Team, based in Shepton Mallet (BA4 5BT). This is a locum role paying £27.78–£38.00 per hour via Prism, available from 20 July 2026.

This post focuses on supporting young people with learning disabilities as they move from children’s into adult social care services. You will carry a caseload of varying complexity, undertaking holistic needs and risk assessments, developing person-centred care plans, and working in partnership with health, education, housing, and voluntary sector agencies to promote independence and safeguard vulnerable adults.

Key responsibilities include: completing statutory reviews and assessments in line with relevant legislation; managing cases presenting in crisis; engaging with people who decline services but remain at risk; chairing and contributing to multi-agency meetings; supervising unqualified or vocationally qualified staff; and maintaining high-quality records on council IT systems. The role requires lone working in community settings, schools, and client homes, and a vehicle is essential.

Applicants must hold a recognised social work qualification (BA or MA Social Work or equivalent) and be registered with Social Work England. Experience of transition work or working with young people with learning disabilities is desirable.
