Job Description
Caerphilly County Borough Council is recruiting a qualified Social Worker to join its Adult Services – Older People team, based at The Rhymney Integrated Health and Social Care Centre, Rhymney (NP22 5PW). The rate is £30.00–£39.58 per hour, with a start date of 27 July 2026.
This role sits within the North locality of the Older People section and focuses on assessment and care management, working to maximise and maintain the independence and dignity of adults requiring care and support. You will carry out needs and risk assessments, develop and review care and support plans, respond to crisis situations, and work within multi-disciplinary teams alongside health colleagues including Community Nurses and Community Psychiatric Nurses.
Key responsibilities include preparation for and direct work with individuals, families, carers, groups and communities; advocacy; participation in decision-making forums; safeguarding adults; and joint working with the LHB Complex Care Team on nursing-home reviews. You may occasionally deputise for the Senior Social Worker or Team Manager, and may mentor Review Officers, students and newly appointed staff.
The role requires registration as a Social Worker with Social Care Wales (or an equivalent regulator in England, Scotland or Northern Ireland), a Diploma or Degree in Social Work, knowledge of the Social Services and Well-being Act (Wales), Mental Health Act, and Mental Capacity and Deprivation of Liberty legislation, plus experience of assessment and care planning in a community setting. A full UK Category B driving licence and use of a business-insured vehicle are required. Enhanced DBS with Adults Barred List check required.
