Job Description

Caerphilly County Borough Council is seeking a qualified Social Worker to join its Adult Services division, specifically within the Older People section based at The Rhymney Integrated Health and Social Care Centre, Rhymney, Caerphilly (NP22 5PW). The role is offered via Prism at a rate of £30.00–£39.58 per hour, starting 27 July 2026.

You will deliver assessment and care management services to adults requiring care and support, helping them to maximise and maintain their independence and dignity. Key responsibilities include conducting needs assessments and reviews, devising care and support plans, managing risk, multidisciplinary working with health colleagues (including Community Nurses and Community Psychiatric Nurses), and ensuring integration of service provision with the Local Health Board. You may also be required to deputise for the Senior Social Worker or Team Manager and to mentor students and newly appointed staff.

The role involves agile, office, and community-based working, including lone working in service users’ homes. Occasional evenings and weekends may be required. An Enhanced DBS with Adults Barred List check is required.

Candidates must hold a Diploma or Degree in Social Work and be registered with Social Care Wales (or an equivalent regulator in England, Scotland, or Northern Ireland). Knowledge of the Social Services and Well-being Act (Wales), Mental Health Act, Mental Capacity Act, and DoLS legislation is essential. A full UK driving licence and access to a vehicle insured for business use are required. Welsh language skills are desirable.
