Job Description
An exciting opportunity has arisen for a qualified Social Worker to join the New Malden Hub, one of three adult locality teams within the Royal Borough of Kingston’s Adult Social Care service. Based primarily at Guildhall 2, Kingston upon Thames, the team attends the office two days per week for team meetings, peer support, and networking.
The team supports adults aged 18 and above with physical health and social care needs, with a primary client base of older adults and people with physical disabilities. You will carry a varied caseload encompassing assessment, support planning, safeguarding investigations, and ongoing case management, all delivered through a strengths-based, outcome-focused approach.
Key responsibilities include facilitating assessments and support planning in accordance with the Care Act 2014 and Mental Capacity Act 2005, undertaking statutory reviews within required timescales, and working closely with the hospital social work team to facilitate discharges via Discharge to Assess pathways. You will also liaise with the Reablement service, consider funding pathways (ASC, CHC, Section 117), and engage with multi-agency partners across health and social care.
Candidates must hold a recognised Social Work qualification (undergraduate or postgraduate), be registered with Social Work England, and have the legal right to work in the UK. Strong assessment, case management, and communication skills are essential, along with sound knowledge of relevant legislation, safeguarding procedures, Personal Budgets, and Self-Directed Support.
