Job Description
Somerset Council is seeking an experienced Adults Social Worker to join a dedicated Review Project focused on adults with learning disabilities, based in Taunton (TA1 4DY) with a minimum of two days per week in the office and travel across Somerset. This is a hybrid interim role paying £27.78–£38.00 per hour, starting 17 August 2026.
The role centres on undertaking detailed Care Act reviews and reassessments for adults with learning disabilities. You will complete face-to-face visits across a range of settings — including people’s homes, supported living, and residential environments — gathering and analysing evidence from multiple sources to inform professional decision-making. A key element of the role involves identifying eligible care and support needs, producing audit-ready assessments and support plans, and distinguishing between social care needs and NHS/health responsibilities.
You will also identify cases where Continuing Healthcare (CHC), Checklist consideration, or challenge may be appropriate, and participate in multidisciplinary funding discussions and CHC-related processes, providing clear professional analysis and recommendations.
Essential requirements include a recognised social work qualification (BA or MA in Social Work or equivalent), current registration with Social Work England, significant post-qualifying Adult Social Care experience, and strong Learning Disability practice experience. Experience completing Care Act reviews and reassessments, strong analytical and assessment skills, professional judgement, and the ability to work autonomously are all essential. A vehicle and driving licence are required for this role.
If you are an experienced adults social worker with strong LD and CHC knowledge, please apply now.
