Job Description

The role will be primarily to review people who have been discharged from hospital under discharge to assess, assessing homecare, care home placements, undertaking CHC funding meetings. May also involve work for Older Peoples Mental health team.

Role will mean that need to be present in Middlesbrough on a daily basis in order to visit clients.
