Job Description

Are you a qualified Social Worker looking for a dynamic, impactful role in adult services? Blackburn with Darwen Council is seeking a motivated Adults Social Worker to join their Hospital Discharge and Discharge to Assess (D2A) service, based at 10 Duke Street, Blackburn.

This is a fast-paced, hybrid role focused on facilitating timely, safe, and person-centred discharges from hospital into community settings via established D2A pathways. Working within a multi-disciplinary team, you will ensure individuals leave hospital with the right support in place to recover and regain independence in the most appropriate environment.

Key responsibilities include undertaking strengths-based assessments in line with Care Act principles, facilitating safe hospital discharges through the D2A pathway, collaborating with health professionals, families, and community services, developing and reviewing support plans, safeguarding vulnerable adults, and maintaining accurate case records.

The service operates on person-centred, strengths-based principles, prioritising what matters most to each individual and keeping their voice central to all decision-making.

Candidates must hold a recognised Social Work qualification and current Social Work England registration, with experience in adult social care — ideally within hospital discharge or community settings. Strong communication skills, confident decision-making, and the ability to manage competing priorities in a fast-paced environment are essential.

This is a contract position paying £30 per hour, commencing 1st June 2026 through to 30th June 2026, with hybrid working arrangements available. An ASAP start is preferred. Apply now to make a meaningful difference.
