Job Description

An experienced Advanced Practitioner is required to join Blackburn with Darwen Council’s North and East Integrated Neighbourhood Team, based at Barbara Castle Way Health Centre, Blackburn. This is a temporary role running from 14 September 2026 to 12 February 2027, paying £32 per hour.

Working within an integrated neighbourhood model, you will support the Team Manager with the day-to-day running of the team and deputise as required. Core responsibilities include undertaking complex Care Act assessments, managing support planning, evaluating commissioned care packages, and leading on safeguarding enquiries under s42. You will also represent adult social care in multi-agency settings alongside health professionals, managing funding pathways and integrated care needs.

A key element of the role is leadership and practice development — mentoring social workers, assessment officers, and review staff, and providing reflective supervision and healthy challenge to shape service development. The team operates within a person-centred, strengths-based framework, working in close partnership with health, voluntary, community, and faith sector organisations.

Applicants must be qualified social workers registered with Social Work England, with demonstrable experience in adult social care, Care Act assessments, and safeguarding. A positive attitude to transformation and innovation is essential.
