Job Description

An outstanding opportunity has arisen for an experienced Adults Advanced Practitioner to join Somerset Council’s Adult Social Care directorate, based full-time on-site at Musgrove Park Hospital in Taunton. This is a senior, clinically and professionally demanding role offering genuine leadership impact across a busy hospital and community interface setting.

Reporting directly to the Service Manager, you will provide high-quality professional leadership to a team of up to seven qualified social workers, offering supervision, appraisals, coaching, and mentoring. You will hold oversight of complex cases, chair key meetings, and support staff with statutory decision-making under the Care Act 2014. The role also carries delegated budget authority for personal budgets and devolved team expenditure.

Key responsibilities include leading safeguarding adults practice, supporting timely hospital discharge and preventing unnecessary admissions, and promoting independence-focused, personalised assessment and support planning. You will contribute to service development, quality audits, Court of Protection applications, and performance monitoring at local and national levels.

Candidates must hold a recognised social work qualification, be currently registered with Social Work England, and bring extensive experience in complex adult casework alongside proven staff supervision and management skills. Post-qualifying study or equivalent CPD is desirable. Management experience is essential.
