Adult Social Worker – Localities Team

Position Overview: 

As an Adult Social Worker within locality, you will play a vital role in assessing and reviewing the support being provided to older adults in the community and those in 24-hour care. Your primary responsibility will be to complete Care Act compliant assessments in a timely manner, ensuring the individuals’ needs are met effectively. 

Responsibilities: 

Conduct comprehensive assessments and reviews of adults with care and support needs, considering their physical, emotional, and social needs. 

Determine ongoing eligibility for services and support, adhering to the guidelines set out in the Care Act. 

Adhere to the principles of the Mental Capacity Act and implement best interest processes where required.  

Collaborate with individuals, their families, and other healthcare professionals to gather relevant information, helping them to understand their options and make informed decisions when developing comprehensive care plans,

Advocate for individuals’ rights and entitlements, ensuring that they receive appropriate and timely support services. 

Maintain accurate and up-to-date records and documentation in accordance with established policies and procedures. 

Stay current with changes in legislation, policies, and best practices related to social work, care assessments, and community support services.     

 

Requirements: 

Bachelor’s or Master’s degree in Social Work 

Valid social work registration. 

Valid driver’s license and own car with business insurance 

Proven experience in conducting assessments and developing care plans. 

Experience in Adult community social work is essential

Sound knowledge of the Care Act, Mental Capacity Act and Best Interest Processes is essential for this role.  

Knowledge and experience of Continuing Health Care processes and participating in DST’s would be an advantage, but not essential.

You must have:

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
