Job Description

Please be aware this is a Long Term Temporary post to start ASAP.

Integrated Community Coordination Teams – Whilst working in the ICCT¿s you will be completing strengths-based Care Act assessments and person-centred support planning with a wide variety of service user groups aged 18 and over. The work is diverse, and the aim is to promote the wellbeing of adults with care and support needs to continuously improve their outcomes, creating lasting connections with their local community.

The following is essential criteria:

– Degree in Social Work

– SWE Registration

– Right to work within the UK

Please be aware this is a Long Term Temporary post to start ASAP.

You must have:

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
