The Role:
To do everything possible to ensure the team provides an efficient and effective service to the residents of Bolton. This involves detailed assessment of need (Care Act) and effective asset based support planning. Taking into account available resources, universal and voluntary services, carers assessments, capacity assessment, best interest decision making and continuing health care assessments.

Inter disciplinary and multi agency working as part of a Integrated Care Partnership in Bolton.
Preferred experience of resettlement from out of area placements back into the local area, working with complex cases, forensic care pathways, specialist provider services, devising care packages and undertaking detailed assessments of support.
Supporting young adults moving into Adult Services, supporting individuals to move on from their family homes, and involvement with Safeguarding investigations.

About You:
We are looking to recruit an enthusiastic and motivated individual who has a passion for working with adults with Learning Disabilities, understanding their needs, aspirations and supporting them to enhance their quality of life.
You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and decision making.
You will be able to remain focused and motivated during current remote working arrangements that are in place.
These posts are subject to enhanced clearance from the Disclosure and Barring Service and registration with Social Work England.

We are a large multidisciplinary team involved in complex areas of work and so there are many opportunities to be involved in formal decision making forums and court processes.
There is an expectation that there will be an initial period of training and for them to be in the office for the first 2 weeks.

Following that the expectation would be to be office based for 50% of the time, with an understanding that they could be available to come into the office beyond that if required.
This role will involve attendance at Community Treatment Reviews and require travel to placements out of borough.

The advert will be for 2 social workers for the Intensive Support Team which sits under the umbrella of the community learning disability team.

You must have:

  • A professional qualification in social work and experience of working with Adults
  • Social Work England registration.
  • Post qualified experience.
  • References required for the last 5 years of employment.
