Job Description

Somerset Council is seeking a qualified Adults Social Worker to join the Transition to Adult Social Care Team, based in Shepton Mallet (BA4 5BT). This is a locum role paying £27.78–£38.00 per hour via the Prism portal, available from 24 August 2026.

This post sits within a team supporting young people moving from children’s to adult services, with a particular focus on Learning Disabilities. You will hold a caseload of varying complexity, undertaking needs-led assessments of risk, care needs, and carers’ needs in accordance with relevant legislation, policy, and procedure. Work is carried out across a variety of settings — including schools, office environments, and client homes — requiring effective lone-working practices and strong multi-agency collaboration with health, housing, education, and voluntary-sector partners.

Key responsibilities include completing statutory reviews, developing and reviewing care plans, negotiating care provision, and managing people presenting in crisis. You may also supervise unqualified or vocationally qualified staff or students, act as an appropriate adult in complex cases, and make applications to the Court of Protection where appropriate.

Essential requirements: a recognised Social Work qualification (BA or MA in Social Work or equivalent), current registration with Social Work England, and access to a vehicle. Experience working with young people with Learning Disabilities and those transitioning into adult services is highly desirable.
