Adults Social Worker – Hospital Discharge Team – #5299438

Job Description

Semester Recruitment is seeking a qualified Adults Social Worker for Newcastle City Council, based at Westgate Community College, NE4 9LU. This is a community-facing post-discharge role supporting adults 4–10 weeks following hospital discharge, with a mix of office and home-based working due to community appointments.

The role involves holistic assessment and support planning for adults in the post-discharge period, carrying out Mental Capacity Act assessments, safeguarding enquiries as appropriate, and involvement in Continuing Healthcare (CHC) processes.

The successful candidate must be a qualified Social Worker registered with Social Work England and have demonstrable experience in adults’ services, ideally including hospital discharge, CHC, MCA, and safeguarding adults. Experience of community-based assessment and support planning is essential.
