Job Description

Caerphilly County Borough Council is seeking an experienced Adults Senior Practitioner to join its Community Mental Health Team, based at The Rhymney Integrated Health and Social Care Centre. This is a multi-disciplinary post within Adult Services, delivering community mental health provision across Caerphilly County Borough.

Working within the Care and Treatment Planning framework, you will assess individual needs, coordinate and deliver planned care and support to people requiring a Community Mental Health Team response, and manage risk across a complex caseload. A key component of the role is fulfilling AMHP duties in accordance with the Mental Health Act 1983 and the Code of Practice, and you will participate in the daytime AMHP rota. You will also contribute to the development of less experienced colleagues and enable learning for AMHP and social work students.

The team works closely with Aneurin Bevan University Health Board, statutory agencies including housing, police and probation, and third-sector organisations to coordinate care and achieve positive outcomes for service users and carers.

Essential requirements: Diploma or Degree in Social Work (or equivalent); registration as a Social Worker with Social Care Wales (or equivalent UK regulatory body); Approved Mental Health Professional qualification; minimum two years’ post-qualifying experience in adult mental health social work; demonstrable experience of AMHP practice and risk assessment. Knowledge of the Social Services and Wellbeing Act 2014 and Mental Health (Wales) Measure is required.
