Job Description
Are you passionate about empowering young people as they take their first steps into adulthood? Bury Council’s Preparing for Adulthood (PFA) Team is seeking an experienced Adults Social Worker to support young people aged 14–25 with care needs, disabilities, and complex transitions into adult life.
You will hold a caseload of transition work, driving excellent transitions planning aligned with the national Preparing for Adulthood framework. Key responsibilities include building effective partnerships across services, schools, colleges, and health; championing young people’s voices and aspirations; and modelling strengths-based, person-centred, co-productive approaches that maximise independence and inclusion.
The team works closely with Children’s Services, Education, Health, and SEND, benefiting from a values-driven culture built on kindness, collaboration, and trust. You will receive regular supervision, access to high-quality training, leadership development opportunities, and flexible hybrid working arrangements.
Ideal candidates will be qualified social workers with experience in adult social care, transitions work, or disability services. You must be organised, solution-focused, and able to collaborate across agencies with clarity and purpose.
