Job Description

Are you passionate about making a lasting difference to the lives of young people transitioning out of care? We are seeking a dedicated Leaving Care Worker to join a Children’s Services team, acting as a pivotal point of contact and coordination for young people aged 16 to 25 as they move into adulthood.

In this role you will provide flexible, reliable support to care leavers, coordinating pathway plans and bringing together resources from partner agencies to ensure young people transition safely and healthily. You will carry out direct work with young people and their wider networks — including peers and family — to help them achieve outcomes aligned to their individual plans and the Council’s Children in Care and Care Leavers Strategy.

Key responsibilities include: supporting young people to secure education, training, and employment; facilitating access to stable accommodation; delivering targeted interventions for those at elevated risk (including anti-social behaviour, mental health challenges, or risk of losing housing or employment); and engaging harder-to-reach young people through creative relationship-building. You will also contribute to a duty service, providing crisis support and information, advice, and guidance.

The leaving care offer spans both children’s and adults’ services (16–25), so a working knowledge of both arenas is essential. You will need the confidence to challenge families, agencies, and wider relationships constructively to drive improved outcomes for care leavers.

The ideal candidate will have experience working with young people in a care-leaving or similar context, strong multi-agency working skills, and a sound understanding of the risks and vulnerabilities faced by care leavers.
