🚑 Adult Mental Health Social Worker – Ward Discharge & Community Care

Long-term temp assignment · 36 hrs pw · Mon–Fri · Competitive hourly rate

What you’ll be doing

  • Turbo-charge ward discharges 🏥 – complete strengths-based Care Act assessments on the ward, draft support plans and secure rapid, safe exits.

  • Unlock funding 💷 – write clear business cases, s117 agreements and Continuing Healthcare submissions to secure the right packages first time.

  • Safeguard & advocate 🛡️ – chair risk meetings, prepare CoP DoL / 21a reports and give confident evidence if required.

  • Keep carers in the loop 🤝 – complete Carer assessments and connect families to support that really helps.

  • Rotate onto duty 🔄 – cover the duty line on a rota, triage new referrals and offer rapid social-care advice across the MDT.


What you’ll bring

  • SWE-registered Social Worker (CQSW/DipSW/BA/BSc).

  • Solid post-qualifying experience in adult mental-health social care – ward or community.

  • Up-to-date know-how on the Care Act 2014, Mental Capacity Act 2005 & Adult Safeguarding.

  • Confident writer of court-standard reports and funding applications.

  • Full UK driving licence & own car 🚗 (mileage paid).
