Are you a passionate and skilled social worker with experience in hospital settings? Wakefield Council is seeking a dedicated Senior Social Worker to join our Hospital Social Work Team, helping patients and their families navigate the complexities of hospital discharge and post-care services.
About the Role:
As a Senior Social Worker within our hospital team, you’ll play a vital role in providing high-quality social work services to patients, families, and healthcare professionals. You’ll work in collaboration with multidisciplinary teams, ensuring safe and effective discharge planning, while advocating for the needs and rights of individuals and promoting their well-being.
This is a rewarding and varied role that requires strong leadership, critical thinking, and a person-centred approach to care. You’ll manage complex cases, support and mentor junior colleagues, and work to ensure that patients receive the appropriate care and services after leaving the hospital.
Key Responsibilities:

Discharge Planning & Assessment: Manage a caseload of patients requiring social work intervention for safe and effective discharge.

Collaboration: Work closely with doctors, nurses, occupational therapists, and other professionals to ensure integrated care and support for patients.

Leadership: Provide support, guidance, and supervision to junior staff within the team, promoting best practice and continuous improvement.

Advocacy & Safeguarding: Ensure the voice of the patient is heard and that all care decisions are made in their best interests, with a focus on safeguarding and welfare.
What We’re Looking For:

Experienced Social Worker with a strong background in hospital or healthcare settings.

Leadership Skills: Demonstrated experience in supervising and supporting social work staff, fostering a supportive team environment.

Person-Centred Approach: Passion for advocating on behalf of vulnerable individuals, with a focus on patient dignity and autonomy.

Excellent Communication: Ability to build effective relationships with patients, families, and multidisciplinary teams.

Adaptability & Problem-Solving: Ability to manage complex cases and respond flexibly to changing circumstances.

You must have:

A professional qualification in social work

Social Work England registration.

Post qualified experience.

Enhanced DBS/Update service.

References required for the last 5 years of employment.
