Job Description

Please be aware this is a Long Term Temporary post to start ASAP.

The following is essential criteria:

– Degree in Social Work

– SWE Registration

– Right to work within the UK

2 agency Social Work posts are available within the Transfer of Care Hub (Hospital Discharge Team) at Warrington General Hospital. This post is predominantly based on-site with some hybrid working available. 

The successful candidate will be required to have access to their own vehicle as some visits to out-of-area hospitals are required. 

The ideal candidate will have experience in hospital discharge work, or working within an integrated team. The candidate MUST have a minimum of 2 years experience in statutory Social Work.

• A professional qualification in social work

• Social Work England registration.

• Post qualified experience.

• Enhanced DBS/Update service.

• References required for the last 5 years of employment.
