Job Description

An exciting opportunity for a qualified Team Manager to lead the Young Adults Team within Lancashire’s Learning Disabilities & Autism service, supporting young people aged 14-25 as they transition from Children’s to Adult Social Care. You will provide operational leadership and management oversight to a multidisciplinary team, driving high-quality Care Act assessments, care planning and reviews, and ensuring safe, timely and effective transitions into adulthood. Key duties include supervision, performance management and workforce development, promoting multi-agency collaboration with Children’s Services, SEND, Health and Education, and maintaining oversight of safeguarding activity and high-risk cases. You will also contribute to service improvement, audit and quality assurance, support allocation planning and strategic forecasting, and manage budgets to ensure value for money while maintaining person-centred outcomes. Candidates must be a Qualified Social Worker registered with Social Work England, with strong knowledge of the Care Act 2014, Mental Capacity Act and safeguarding legislation, and experience of strengths-based practice with young adults with complex needs.
