Job Description

Somerset Council is seeking an experienced Adults Social Worker to join a dedicated Learning Disabilities Review Project based in Taunton (TA1 4DY). This is a hybrid role requiring a minimum of two days per week at Somerset Council offices, with regular travel across Somerset to undertake face-to-face visits in community, supported living, and residential settings.

The role centres on undertaking comprehensive Care Act reviews and reassessments for adults with learning disabilities. You will gather and analyse evidence from multiple sources, determine eligibility, and produce audit-ready assessments and support plans that reflect strengths-based, person-centred practice. A key element of the role involves identifying and progressing cases where Continuing Healthcare (CHC), Checklist consideration, or challenge may be appropriate, and participating in multidisciplinary funding discussions and review processes.

Essential requirements include a recognised Social Work qualification (BA or MA in Social Work or equivalent), current Social Work England registration, and significant post-qualifying experience in Adult Social Care with strong Learning Disability practice. You must have demonstrable experience completing Care Act reviews and reassessments, excellent analytical and assessment skills, and the ability to work autonomously and manage a demanding workload to project timescales. A vehicle and full driving licence are essential. Knowledge or experience of CHC, DSTs, and health/social care funding boundaries is highly desirable.
