Job Description

Blackburn with Darwen Council is seeking an experienced Adults Social Worker to join a busy Discharge to Assess (D2A) team, supporting BwD residents within assessment beds across the local area. This is a Monday to Friday role (9am–5pm) based at 10 Duke Street, Blackburn, paying £30 per hour. The contract runs from 31 August to 6 November 2026, with the possibility of extension.

Working collaboratively with health professionals, patients, families, and community services, you will assess ongoing care and support needs following discharge from acute hospital settings. Key responsibilities include completing strengths-based social care assessments within D2A bed settings, facilitating safe and effective discharge planning, undertaking Care Act assessments and reviews, coordinating support packages and community-based services to maximise independence, contributing to safeguarding enquiries and risk assessments, and maintaining accurate, timely case records.

You will need to be a qualified Social Worker registered with Social Work England, with previous experience in hospital discharge, intermediate care, or adult social care. Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding procedures is essential, as is the ability to work effectively within a fast-paced multidisciplinary environment. A car is required for this role.
