Job Description

The palliative care social work team, work collaboratively with Health to support adults (18+) and their family and carers who reside across Calderdale and Kirklees (if health funded with a Calderdale GP).

Social workers within this service provide a vital role in ensuring that statutory duties are met under the Care Act, to provide support to people who meet the Care Act national criteria for adults requiring support due to an end of the life prognosis.

The role will primarily work with individuals and their families / carers who require support with discharge planning from hospital where NHS fast track funding has been agreed, due to a rapidly deteriorating health condition. The team will work closely with the hospital discharge team and the Integrated care board to complete appropriate assessments and coordinate services for timely and safe discharges. 

The team also work with individuals and their families / carers in the community and local hospice, where they have a palliative prognosis of less then 12 months. Tasks include Care Act Assessments in line with Mental Capacity and Human Rights legislation, risk assessments, continuing health care checklists and Decision Support Tool meetings. Safeguarding enquiries under s42 Care Act.
