Adults Advanced Social Work Practitioner – Hospital Team
Location: Barnsley
Working Pattern: 3 days per week onsite in the office
We have an exciting opportunity for a qualified Advanced Social Work Practitioner to join our Hospital Social Work Team, working in close partnership with health professionals across various discharge pathways.
What we’re looking for:

Essential: Hospital social work experience within adults and strong knowledge of discharge processes.

Essential: Experience managing and supervising social work staff.
-
Skilled in collaborative working with health partners to ensure safe, timely discharges.

Strong IT skills, including Liquid Logic and Excel.

In-depth understanding of:
-
Mental Capacity Act 2005
-
Human Rights Act 1998
-
Care Act 2014
-

Ability to work with complex needs such as substance misuse, homelessness, mental capacity issues, self-neglect, and advocacy.

Confident in staff supervision, decision-making, and caseload management.

Ability to undertake Care Act, Capacity, and Best Interest Assessments when required.
What you’ll do:

Lead and support the hospital social work team.

Attend daily meetings with health partners.

Balance individual needs with health care pressures in line with integrated care board partnerships.

Carry a small caseload alongside line management responsibilities.

Post qualified experience.

Enhanced DBS/Update service.

References required for the last 5 years of employment.
