Adults Advanced Social Work Practitioner – Hospital Team

Location: Barnsley
Working Pattern: 3 days per week onsite in the office

We have an exciting opportunity for a qualified Advanced Social Work Practitioner to join our Hospital Social Work Team, working in close partnership with health professionals across various discharge pathways.

What we’re looking for:

  • Essential: Hospital social work experience within adults and strong knowledge of discharge processes.

  • Essential: Experience managing and supervising social work staff.

  • Skilled in collaborative working with health partners to ensure safe, timely discharges.

  • Strong IT skills, including Liquid Logic and Excel.

  • In-depth understanding of:

    • Mental Capacity Act 2005

    • Human Rights Act 1998

    • Care Act 2014

  • Ability to work with complex needs such as substance misuse, homelessness, mental capacity issues, self-neglect, and advocacy.

  • Confident in staff supervision, decision-making, and caseload management.

  • Ability to undertake Care Act, Capacity, and Best Interest Assessments when required.

What you’ll do:

  • Lead and support the hospital social work team.

  • Attend daily meetings with health partners.

  • Balance individual needs with health care pressures in line with integrated care board partnerships.

  • Carry a small caseload alongside line management responsibilities.

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
