Please be aware this is a Long Term Temporary post to start ASAP.

attend ward reviews to work as part of mental health multi-disciplinary teams in adults and older persons wards to complete assessments of need under the Care Act and develop strengths based support plans. – key in developing discharge plans and lead on these – complete Mental Capacity Assessments and make Best Interest decisions when required – partnership working with health, housing, children’s services etc essential The post holder needs to be a strong communicator and have a robust understanding of the Care Act 2014, the Mental Capacity Act 2005 and the Mental Health Act 1983 in practice. They will complete section 117 discharge planning meetings and Social Circumstance Reports for mental health tribunals. They will conduct section 42 safeguarding enquiries when required. The team cover Merseycare wards across the region. Based currently in Broadoak Unit, Liverpool.

The following is essential criteria:

– Degree in Social Work

– SWE Registration

– Right to work within the UK

You must have:

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
