About the Role
We have an exciting opportunity for an Advanced Social Work Practitioner to join our Hospital Social Work Team in Barnsley. You will work in partnership with health professionals across various discharge pathways to ensure safe and timely patient flow.
The ideal candidate will have prior experience at Advanced Practitioner level, with strong knowledge and skills in managing adult social care staff. You must demonstrate a thorough understanding of the legislative frameworks underpinning social work practice and be confident in supporting staff with complex case management and critical decision-making.
Key Responsibilities

Work alongside another Advanced Practitioner and Team Manager to manage a team of 22 Social Workers and Assistant Social Care Practitioners.

Oversee day-to-day team operations, including:

Monitoring incoming hospital referrals

Allocating work promptly
-
Responding to staff queries
-
Attending daily meetings with health partners
-
Conducting timely staff supervision
-
Auditing and authorising Care Act assessments and support plans
-
Managing adult social care resources and budgets effectively

Maintain a small caseload, working with individuals with complex health and social care needs, including substance misuse, homelessness, mental capacity issues, self-neglect, and advocacy.

Balance individual needs with assessed social care requirements and healthcare pressures in line with integrated care board partnerships.
Essential Requirements

Recognised Social Work qualification and registration with Social Work England (or awaiting registration).

Clear and full DBS clearance.

Previous experience in Hospital Discharge and ideally within a Hospital Social Work / Integrated Care setting.

Strong understanding of discharge planning within an acute hospital trust.

Ability to lead and support staff in complex case management and decision-making.
Desirable Skills & Experience

Experience working with adults with complex needs, including substance misuse, housing issues, mental capacity concerns, and safeguarding.

Knowledge of integrated health and social care systems and processes.
Additional Information

Onsite Requirement: Minimum 3 days per week onsite to attend meetings and work collaboratively with the team.

Hybrid working policy applies for remaining days.

A professional qualification in social work

Social Work England registration.

Post qualified experience.

Enhanced DBS/Update service.

References required for the last 5 years of employment.
