Please be aware this is a Long Term Temporary post to start ASAP.

The following is essential criteria:

– Degree in Social Work

– SWE Registration

– Right to work within the UK

A qualified social worker is required for the hospital discharge to assess team South. The post is based in Wythenshawe hospital, south Manchester and involves supporting patients in both Wythenshawe hospital and Trafford General with arranging safe and timely discharges back to their own home or 24 hour care setting. 

Please be aware this is a Long Term Temporary post to start ASAP.

You must have:

  • A professional qualification in social work

  • Social Work England registration.

  • Post qualified experience.

  • Enhanced DBS/Update service.

  • References required for the last 5 years of employment.
