Job Description

Trafford Council is seeking an experienced Social Worker for its Interim Triage Team within the Adult Social & Prevention Hub, based at Trafford Town Hall. This fast-paced Front Door service provides timely, strengths-based and person-centred responses to adults, carers and professionals seeking support.

Key duties include completing proportionate strengths-based assessments in line with the Care Act 2014, undertaking capacity assessments and Best Interest decisions under the Mental Capacity Act 2005, managing a caseload of adults at risk of harm or neglect, and safeguarding adults within relevant policy frameworks. You will work as part of a multi-disciplinary team, liaising with health and social care professionals and partner agencies, and contribute to project groups improving service delivery.

You will need a recognised Social Work Qualification, registration with Social Work England, experience working with adults in a social care and multi-disciplinary setting, and sound knowledge of the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983. A full driving licence is required as home visits will be needed following transfer to the new hub. Enhanced DBS with Adults’ barred list check required.
