Care Management Assistant – Coordination and Review Team – #OR113191

August 26, 2026
£18 - £23 / hour

Job Description

Bromley Council is seeking experienced Locum Social Care Practitioners (Care Management Assistants, BR8/9) to join the Adult Social Care Coordination and Review Team based at Churchill Court, Bromley. The role is offered at £18.03–£23.47 per hour, starting 5 October 2026.

Working under direction and guidance, you will carry out planned Care Act needs assessments and annual reviews for adults receiving funded support across a range of settings, including care homes, Shared Lives schemes, Extra Care Housing, domiciliary care, and residential placements — including visits outside the borough where required.

Key responsibilities include completing proportionate Care Act assessments and reviews, developing and monitoring support plans, promoting Direct Payments and choice and control, undertaking Mental Capacity Assessments and contributing to Best Interest decision-making, collaborating with care providers, housing, and health professionals, and maintaining accurate, timely case records in line with statutory requirements. You will also support preliminary safeguarding enquiries and participate in duty as needed.

Essential requirements include recent experience in adult social care (ideally older people and/or housing-based services), working knowledge of the Care Act 2014 and Mental Capacity Act 2005, and the ability to manage a complex caseload autonomously. A relevant health and social care qualification (e.g. NVQ) is preferable but not essential at BR8. A satisfactory DBS check is required and you must be able to travel across the borough.

This role does not require qualified social worker status. If you are an experienced social care practitioner ready to make a real difference to adults in Bromley, please apply now.
